This Licensed Practical Nurse position is located in Nursing Service at the White River Junction, Vermont location. This position is a Full time at 40 hours per week. Please see the "What To Expect Next" area of the announcement. This Licensed Practical Nurse position is located in Nursing Service at the White River Junction, Vermont location. This position is a Full time at 40 hours per week. Accurately administers and records medications and treatments. Medication administration includes the IV route according to policy and under the supervision of the RN. This role includes determining need, contra-indications, allergies, and identifying and recording reactions/interactions. The age specific (the patient age less than 65 years and 65 years and older) and pharmacological implications are considered and applied during medication administration. Demonstrates knowledge of the nursing process in administering patient care by collecting data and collaborating with the interdisciplinary team on the care plan development, implementation and ongoing evaluation and revision of plan of care. Recognizes age-specific variations of clients and makes appropriate interventions. Identifies emergencies and intervenes appropriately. Performs nursing procedures and therapies according to policies and procedures. Performs technical nursing skills such as complex dressing changes and care of various tubes and drainage systems. Identifies potential factors that would impede healing and intervenes to prevent infection and promote healing. Utilizes the nursing process with the knowledge of physiological, pharmacological, psychological, sociological principals and considers age specific variation and developmental needs. Performs support duties for diagnostic and therapeutic procedures which include preparing the patient, providing nursing care associated with the procedure and monitoring the patient's condition. Functions as a member of the interdisciplinary team in the development, implementation, and evaluation of the patient's treatment and discharge plan. Contributes to the rehabilitation of the patient by providing restorative care in collaboration with physical and occupational therapy. Documented consistently utilizing electronic medical record and current patient care forms to include, but not limited to, admission and assessment forms, progress notes, graphic, treatment, and medication sheets, vital signs including Pain as the 5th vital sign. Reports pertinent information to appropriate personnel during the course of their tour of duty. Receives, discusses, and communicates patient information upon transfer or change of tour of duty. Communicates to patients and families the rationale for treatment, diagnostic, and therapeutic procedures. Recognizes unique communication needs of various age groups. Establishes therapeutic relationships with individual patients to elicit feelings and concerns about his/her illness and treatment. Communicates effectively and establishes positive relationships with members of other disciplines. Creates an atmosphere of mutual trust, acceptance, and self-worth. Conducts self with professional demeanor by unconditionally providing basic courteous behavior to the patient and family. Relates to patients and their families by demonstrating respect for their age, culture, and socioeconomic status. Assumes accountability for the quality of nursing care by adhering to the Medical Center Administrative, Personnel and Clinical Policies. This announcement may be used to fill additional vacancies.
