The Program Nurse will be responsible for:
Providing patient care in the inpatient, outpatient and home setting to patients with serious illness and their families
Routine nursing aspects of the program including phone triage and follow-up, patient education and care coordination
To qualify, you must have:
Two year of inpatient (acute care) experience; pediatric experience preferred
20 hours per week
